KMHA Incident Reporting Form
This form is intended for anyone wishing to report a potential breach of the KMHA Code of Conduct or any other incident requiring the association's attention.
Once completed, please email the form to nestor@kamloopsminorhockey.com or conduct@kamloopsminorhockey.com for review and follow-up.
Coach Feedback Form
This feedback form is available year-round and is intended for providing feedback on Head Coaches and Assistant Coaches. Please complete a separate form for each coach you wish to evaluate.
Constructive and honest feedback plays an important role in coach development and will be considered as part of future coach selection and evaluation processes.
Responses are reviewed by KMHA staff and the Director of Hockey Development. If you would prefer that your feedback not be shared with those individuals, please submit it anonymously by emailing conduct@kamloopsminorhockey.com or president@kamloopsminorhockey.com
